3. I suspect that this is a privileges issue - you need to grant dba to the monitoring user in the standby database and connect as sysdba. This is because some of the functions in the monitoring statement, particularly UTL_INADDR, need special privileges to run. Could you please try this and let me know if it resolves the problem.

Bug 9238835 : SELECT UTL_INADDR.GET_HOST_ADDRESS (...) FROM DUAL CAUSES ERRORS IN ALERT LOG

Starting in Oracle 11gR1, a new security measure is introduced for
UTL_INADDR and other PL/SQL network utility packages. The new security
measure is based on XML DB and therefore XML DB is required those PL/SQL
network utility packages. Please see Oracle Database Readme 11g Release 1,
section 32.2.
